General Description:

The Medical and Wellness Coordinator (Nurse) is a very crucial role to the care provided to our population.  The incumbent, (1) oversees the medical, clinical and wellness support provided to Individuals in his or her assigned homes, and (2) is responsible for the daily clinical and administrative functions of the program including ISP monitoring, staff supervision and coordination of all medical and mental health related appointments.  The incumbent is responsible for generating the plan of care, and providing oversight for the medical, psychiatric, dental, physical/occupational therapies, and dietary support needs of assigned Individuals.  The incumbent serves as a liaison to other medical professionals, and trains Direct Care Staff to ensure that said staff are adequately prepared to fulfill duties related to medical and wellness care of assigned Individuals.  The incumbent conducts assessments and makes recommendations to ensure continued wellness support for assigned Individuals.

The incumbent is also responsible for providing crisis intervention and routine medication administration assistance, as well as assisting with all aspects of home life such as meal planning, weight loss management, chronic illness diagnosis, etc.  The incumbent also ensures the safety of each assigned Individual, and promotes and maintains the health of each assigned Individual, as well as ensures that the staff adheres to the goals outlined in the ISP.  The incumbent is required to work collaboratively with other treatment providers, family, and guardians to maximize each person’s dignity and quality of life. This position reports to the Executive Director.

Specific Duties:

  • Ensure the implementation of the ISP as written for all assigned individuals.
  • Ensure that services are provided in compliance with each assigned Individual’s ISP and Behavioral Plan.
  • Develop, implement and manage health programs.
  • Acts as a liaison to health program vendors.
  • Promote and communicate health programs.
  • Analyze health programs and policies to evaluate performance, effectiveness, areas of potential cost reduction, program improvement, or policy change
  • Participate in collection of information related to well-being of residents
  • Monitor and record resident weights
  • Troubleshoot pharmacy and medication supply issues
  • Set-up and manage lists related to resident hospital admittance and discharge, hospice needs and special diet requirements
  • Maintaining health and psych related records for residents
  • Attend ISP meeting for all assigned Individuals, as needed.
  • Participate in the development of the ISP, ISP annual update and ISP revision, as needed.
  • Recommend revisions to a service or outcome in the ISP in accordance with regulatory requirements.
  • Report ISP content discrepancy to Program Specialist, Residential Director, Program Director, and other relevant plan team members.
  • Review the ISP with each assigned Individual in accordance with regulatory requirements.
  • Supervise, monitor and evaluate services provided to the Individual.
  • Report changes related to the Individual’s needs to the Program Director, Supports Coordinator, and relevant plan team members in accordance with regulatory requirements.
  • Coordinate services provided to assigned Individuals in accordance with regulatory requirements.
  • Facilitate amicable relationship between agency and its stakeholders (parents, regulators, supports specialist, programs vendors, etc.).
  • Ensure that incidents are reported in accordance with agency policies, industry standards and regulatory requirements.
  • Handle On-Call issues as assigned.
  • Ensure compliance and completion of the Quality Management Plan
  • Serve on the Peer Review Committee
  • Serve on the Regulatory Compliance Committee
  • Perform related other duties as assigned.


Minimum Qualifications:

  • Licensure: Licensed Practical Nurse, Registered Nurse
  • Minimum of five years of experience working with adults with intellectual and/or developmental disabilities.
  • Minimum of five years of experience working with the 6400 regulations.
  • Thorough knowledge of the 6400 regulations requirement, industry standards, and Every Day Lives Principles. 
  • Ability to perform the essential job functions with or without reasonable accommodation.
  • Preferred Experience: Possession of Train the Trainer Medication Administration Certificate or Train the Trainer CPR, First Aid Certificate

The incumbent is subject to a criminal background check, as stated in the PA Department of Aging and the Older Adults Protective Services Act. The incumbent is also subject to a post offer physical, TB screening, and drug testing.
